Option 2:

Completed and signed claim form

Original certifiied copy of identity document

Proof of bank account details

If the individual is deceased,

Electronic and physical tracing of next of kin / executor

Completed and signed claim form

Original certified copy of death certificate

Original certified copy of deceased's identity document

Original certified copy of the letter of executorship

Original certified copy of executor's identity document

Proof of estate late bank account details

Fee: R430.00 (excluding VAT)

Option 3: Section 37c investigations and recommendations

A typical report will contain the following;

Details of the deceased member

Details of the legal and factual dependants

Details of any nominees

Details of the last will and testament

Any information that will substantiate dependancy

Summaries of any statements

Recommendations

Fee: R200.00 (excluding VAT)
